The short answer
An AI SEO agency works to get your business found in Google and cited or named in AI answers from ChatGPT, Perplexity and Google AI Overviews. PELORA does this work for healthcare: clinics, treatment centers and health brands, nationwide. Our AEO, GEO and SEO program runs $2,000 to $4,000 a month, and retainers start at $3,500.
Last updated . What we checked: OpenAI's crawler documentation and ChatGPT search help page, Google's AI optimization guide, spam policies and helpful content guidance, Bing Webmaster Guidelines and Perplexity's crawler page, each read October 19, 2026, and the PELORA price book.
What does an AI SEO agency do?
An AI SEO agency does classic SEO and then the extra work AI answers need: making sure every AI crawler can read you, that engines see your business as one clear entity, that your pages answer real questions in plain words, and that other trusted sites mention you. Then it measures each engine separately, every month.
That last part separates the real ones from the rest. Google says outright that "No third-party tool has access to our internal ranking or AI systems," so honest measurement means platform reports plus repeated, written-down question tests, not a mystery score. Here is what a PELORA program delivers:

How do you choose an AI SEO agency?
Ask questions that only a team doing the real work can answer well. These ten cover most of it, with what a good answer sounds like.
Which crawlers will you check, and how? A good answer names OAI-SearchBot, PerplexityBot, Googlebot and Bingbot, and says they will test your host and firewall, not just read robots.txt.
How will you measure ChatGPT, Perplexity and Google separately? Look for named reports (Search Console's Generative AI performance report, Bing's AI Performance report) and a written question set run more than once.
Do you report mentions, citations and recommendations as different numbers? They move differently. One blended score hides that.
What will you change off our website? If the answer is "nothing," most of the opportunity is being left alone. AI answers cite other sites as well as yours, such as directories, review sites and press.
Can you show a result with its source? A Search Console screenshot, a dated case study, a published study with its method.
What will you not promise? The right answer includes "a guaranteed citation."
Who writes the content, and who checks the facts? For health, a named clinician should approve claims. Google calls fabricated author profiles "a form of deception."
How many pages will you publish, and why that number? More is not better. Google treats pages built mainly to manipulate rankings as scaled content abuse.
Who owns the content, the listings and the accounts? You should.
How does this connect to bookings? The work should be judged on calls, patients and sales by source, not only on visibility.
What are the red flags when hiring an AI SEO agency?
The biggest red flag is a promise the platforms themselves say nobody can make. Compare what you hear on sales calls with what the engines publish:
What you might hear
What the platform says
"We guarantee you'll rank in ChatGPT."
OpenAI: "Placement is not guaranteed." Bing: "GEO does not guarantee grounding or citations in AI experiences."
"We can get your brand trained into ChatGPT."
No agency can train a model on your business or place you inside its answers. ChatGPT search draws on OpenAI's crawl and partner search providers, and OpenAI says "Placement is not guaranteed."
"You need an llms.txt file and special AI schema."
Google: "You don't need to create new machine readable files, AI text files, or markup to appear in these features."
"We'll publish hundreds of AI pages for you."
Google's spam policy lists "Using generative AI tools or other similar tools to generate many pages without adding value for users."
"Our tool shows Google's AI ranking for you."
Google: "No third-party tool has access to our internal ranking or AI systems."
"Block the AI bots to protect your content."
OpenAI: sites opted out of OAI-SearchBot "will not be shown in ChatGPT search answers." Blocking training (GPTBot) and blocking search are separate choices.
Why does healthcare need a specialized AI SEO agency?
Because health is the category where a wrong sentence costs the most, and Google treats it that way. Its guidance says content on topics that affect people's health "must be highly accurate and consistent with established expert consensus," and it names trust as the most important part of what it looks for.
For a clinic or treatment center, that changes the work in four ways:
Claims are approved, not improvised. Every outcome or treatment claim is checked by your clinician and backed by a source. We do not describe a reader's condition back to them.
Legitimacy is made visible. License details, the medical director's name, accreditation and certifications sit on the page, in plain text an engine can read.
Listings match public records. In our check of 180 licensed California treatment facilities, only 36% could be confidently matched in the federal locator. Mismatched listings make a business harder to verify.
Paid and organic stay consistent. Health ads have their own rules, from LegitScript certification for addiction treatment to Google's healthcare policies. Your organic pages should never say what your ads are not allowed to.
This is personal for us. Our founder built and ran behavioral health treatment programs before PELORA, so we know what a family is checking when they ask an AI about a program at night. For the full healthcare side of our work, see healthcare marketing.
How does PELORA work in the first 90 days?
We baseline first, fix access and identity second, then build answers and mentions, with a checkpoint at 60 days.
The first 90 days of a PELORA AI SEO program. Exact scope is written into your proposal.
Baseline. We write down the 20 to 40 questions your buyers ask, run them in ChatGPT, Perplexity, Gemini and Google, and read your Search Console, Bing Webmaster Tools and analytics. That is the "before" picture every later report compares to.
Access and entity. Crawlers, indexing, snippet settings, then your name, phone and description made consistent everywhere.
Answers. Pages for the questions you are missing, answer first, with sources, reviewed by your team.
Mentions. The directories, associations and publications that engines cite in your field, plus a steady review process.
Report. A monthly, per-engine report in plain English: mentioned, cited, recommended, clicks, and bookings by source.

What owners ask before hiring an AI SEO agency.
How much does an AI SEO agency cost?
At PELORA, search and AI visibility is included in every retainer, starting at $3,500 a month plus a one-time setup fee, and our standalone AEO, GEO and SEO program runs $2,000 to $4,000 a month. The Full Growth Audit, which includes the website SEO, AEO and GEO layer, is $3,000.
What is the difference between an AI SEO agency and a regular SEO agency?
A regular SEO agency aims for rankings and clicks in Google. An AI SEO agency does that and also works on being cited and named inside AI answers: checking AI crawlers, making your business one consistent entity across the web, earning mentions on sites engines cite, and measuring ChatGPT, Perplexity, Copilot and Google's AI features separately.
How long does it take to see results from an AI SEO agency?
Access and indexing fixes show up within days to weeks, and OpenAI says ChatGPT search picks up robots.txt changes in about 24 hours. Being named consistently in AI answers takes months, because it depends on mentions building up across the web. That is why our terms include a 60-day checkpoint.
Should we block AI crawlers from our website?
Usually not the search crawlers. OpenAI says sites opted out of OAI-SearchBot will not be shown in ChatGPT search answers, and Perplexity says PerplexityBot surfaces and links sites in its results. You can block training crawlers such as GPTBot separately and still allow search.
Can an AI SEO agency guarantee results?
No. OpenAI says placement in ChatGPT search is not guaranteed, and Bing says GEO does not guarantee citations in AI experiences. What an agency can promise is the work, the method and honest monthly measurement.
Do you only work with healthcare businesses?
Healthcare is our focus and where our examples and research come from: clinics, treatment centers and health brands. The method works for any business whose buyers research before they buy, and we serve clients nationwide from Newport Beach, California.
Who owns the work if we stop?
You do. Your website content, listings, Search Console and analytics accounts stay yours, and standard terms are a three month minimum, then month to month with 30 days notice.
